This assessment prepares you for your upcoming placement in terms of developing your communication skills, your documentation, and your ability to think like an occupational therapist when reviewing a client’s occupational profile. Students will undertake an interview task in class and then develop an occupational profile of that person. The occupational profile will be documented in a report as per instructions given on vUWS and in class. The report will be typed following all guidelines for writing a health report to professional standards. In week 4 practicums, students will observe as initial interview. This will only be watched once. Recording of the video is not permitted as it is important for occupational therapists to listen to client’s story and record relevant notes. Students will submit their notes from the interview prior to the end of class. These will be referred to when marking the assessment task.

Students will be provided and learn to use the prescribed documentation format. Headings need to align exactly withthe framework. relevant subheadings may be added.

Students will summarise their notes from the interview observed in practicums. This assessment requires students to demonstrate accurate and objective documentation in line with medico-legal requirements. Please review the marking rubric for detailed expectation

The submission will include the following items:

Appendix 1- A completed initial interview template

This must be the template provided on vuws. The template cannot be adjusted or formatted. It is a required skill that students be able to use client records and forms as they are published without adjustment. Students are required to demonstrate their ability to record their findings on an assessment form. The form must be legible, use professional language, objective and accurate to the interview observed in class. This may be typed or handwritten. The completed template needs to align with medico-legal requirements to be considered eligible to be added to a client file.

Appendix 2- Reflection on observing and documenting an initial interview

Students will provide a 400 word reflection on observing and note taking an initial interview. This needs to include:

  • a brief summary of your experience (no more than 100 words)

  • analysis or critique what happened.

  • What worked well (strengths), what didn’t work well (weaknesses) and why do you think this happened?

  • What impact did this have on the overall experience?

  • What would you do differently next time? what should you do more of and what should you do less of in future?

After the class, write your reflection about the interview experience in class . A reflection is a short description with your response to the experience the main focus and will be submitted as appendix 2.
