Assignment #2 involves students creating a digital informed consent and disclosure form for clients. This consent form should be reflective of the Digital Value Statement crafted for Assignment #1 and based on the employment/internship featured in your Digital Value Statement.

Areas to consider while crafting this consent form:

Digital informed consent or expectations/requirements/policy for digital practice

Any state/federal and or practice/organizational/agency standards/rules specific to digital practice?

Any professional code of ethics specific to digital practice?

Process/es for non-compliance/violations/etc.?

Who is to be involved/informed regarding non-compliance/violations/etc.?

Any remediation or training to be available/required etc.?

Be sure to reflect on whether the consent form, process and policies are anti-oppressive, inclusive, and equitable
