Instructions: 

For this part of the summative assessment, you will be creating an infographic using the knowledge gained from the community profiling discussion in week 3 and the formative assignment in week 4.

This will constitute part of the foundation work leading to the development of an annotated bibliography. This will be worth 20% of your final grade.

Consider the key/significant points you want highlighted in your infographic such as:

Your findings/data from the community profile.

A health and wellbeing topic that emerges from the profiling.

Think about the person-centred perspective – and how it relates to your findings.

To create the infographic, you may wish to identify a template or create your own infographic. You will be using the knowledge gained from the community profile and your reading and thinking. The group work, and the identifying common themes will also add to your findings.

Remember you are not ‘sorting out’ the need or topic identified. You are thinking about why the topic is relevant for you, as a nurse, and your context of practice.
