Completing the EBP Model
1. Choose one of the following from the EBP options.
EBP Models: ACE Star Model; Johns Hopkins, or Iowa.
2. Each model has different components that should be completed based on the model template itself. You are responsible for completing those components of your selected model applying your specific POI to those required areas of the model.
3. You are not required to use paragraphs or complete sentences on the model, but it should be complete, legible, and logical to the reader.
Example 1:
-if a section of your model requires you to add the literature used to support your POI, then you should include the titles of the Guideline, Meta-analysis, single studies, etc. used in your prior assignments.
Example 2
-if a section of your model asks you to identify an expected outcome then you should review your outcome from your prior assignments and add that to the model, i.e. "reduce medication errors by 20% on all units"
Models naturally differ so be sure you choose the model that fits your topic and comfort level best.
Implementation Development Appendix Template
You are addressing the additional set of questions/criterion. This must be written using the Implementation Development Appendix Template provided to you in the directions. Do not alter the template.
1. Ask: clinical question must be clearly identified in PICO format. (1 Point)
2. Acquire: List titles of best evidence outlined to answer clinical question and include the levels of evidence used to support implementation from your literature search. (3 minimum.) (2 Point)
3. Appraise: Summarize in 1-2 short bullets the best evidence from each study or guideline/position statement (identified in #2). The bullet items should briefly summarize evidence to support validity and applicability to the problem and situation for each study/guideline. (2 points)
4. Apply: Briefly state at least 2 ethical considerations and why they are important for this population or implementation plan (HIPAA, IRB, i.e. informed consent, etc.) AND 2 bioethical principle considerations and how implementation of your plan impacts the principle (autonomy, beneficence, maleficence, justice). (2 points)
5. Assess: Include 2-3 measures that you will use to determine successful implementation. Measures must include: who, what, when, and how often. Metrics or benchmarks should be used to indicate the standard by which you know the measure has been met, i.e. 20%, 90% pass rate, etc. (1 point)
6. Address potential strengths and barriers. (1 point)
Scholarship/Professional Appearance
1. Model should be free of grammatical, syntax, sentence structure errors.
2. Appropriate headings should be used to match the rubric criteria.
3. The student is permitted to use color and design features as long as they do not detract from the overall professionalism, clarity, and readability of the text/model.
4. The model should be visually appealing to the viewer and fonts should be no smaller than 11 point font. All font should be in Times New Roman or Courier.
5. A title page is not required for this assignment.
6. A reference page is required for the EBP Implementation Appendix.
7. The student does NOT need to submit this assignment to Turnitin.
